North Norfolk is an attractive rural District with a beautiful coastline, fantastic landscapes and big skies within easy reach of the fine City of Norwich, however, some residents need additional help and support to maintain and improve their holistic health and wellbeing. We have two Link Worker posts which offer an exciting opportunity for the successful candidates to deliver a Social Prescribing service which will improve peoples’ lives.

As a Link Worker – Social Prescribing, you will work with residents who need help and support to manage issues in their lives which are having a negative impact on their general health and wellbeing. Our aim is to reduce or prevent the number of unnecessary consultations with General Practitioners and prevent or delay the need for ongoing health and social care services.

The post holders will work collaboratively with partners in the North Norfolk Help Hub as well as a wide range of voluntary and community groups. They will have a regular and visible presence in the GP Surgeries in a designated area enabling them to support the Social Prescribing needs of each Surgery.

The ethos of these posts is to use a person-centred approach, supporting residents to identify their needs and find solutions which allow them to improve their health and wellbeing, this could include accessing existing community groups or supporting the development of new community groups or activities.
