Change of name or address

Please notify the Licensing Team in writing of any of the following changes:

• Change of name (e.g. due to marriage) as shown on operator licence
• Addition/removal of name(s) on operator licence
• Change of applicant (e.g. business transferred to new person(s))
• Change of home address as shown on operator licence
• Change of operator address as shown on operator licence
