Change of name or address
Please notify the Licensing Team in writing of any of the following changes:
- Change of name (e.g. due to marriage) as shown on operator licence
- Addition/removal of name(s) on operator licence
- Change of applicant (e.g. business transferred to new person(s))
- Change of home address as shown on operator licence
- Change of operator address as shown on operator licence
